I'd only replace it if it's gone bad.
I've seen plenty of DIY guys install it upside down, then complain of vibration which could be mis-diagnosed as a bad bearing. I usually check them when the driveshaft is unbolted, just spin it with your hand and feel for any stiffness, roughness, catching or anything other than smooth rotation. To install it right, make sure the thick side is up, with the driveshaft tucked up tight to the underbody.
