Heheh, perhaps an MD'er can step in and elucidate us on this.
But my guess for bypassing the lith is the cost. There could be some side effects of the lith procedure as well, but I'm not sure. Or the doctors could just be lazy and wrote him off home. He's in New York - their ERs must be insanely packed.
