The OEM gaskets at the turbo could be worn. Clamps loosening up will also cause a gasket to blow pre-maturely if there isn't enough pressure between the two surfaces to seal.
The OEM clamp part number have been replaced a couple times so if you're having trouble with them loosening up, they could be defective/worn. If the inner surface of the v-band stretches too much you won't be able to create enough pressure for a constant seal.
