It's the OEM M-Tech rear bumper. You can get it at a dealer, TischerBMW, etc. It's a straight swap, the only thing you will need is new M-Tech bumper guides. The diffuser shown looks to be the LCI stock diffuser.

What are bumper guides? Does the bumper come already equipped with the diffuser? Are there different model diffusers?

the are plastic brackets that help the bumper sides hug the car. You need them

No, you need to order diffuser separately.
