It's usually 1 hole cut in the bed and then a Y-pipe feeding the two stacks. Though I usually don't see them 8-10 inches, I think the "normal" is 5-6 inches, but I could be wrong, as I don't have a diesel truck and I'm only guessing from the sizes I've seen on the road.
