Wast Gate Lever Broken Off

So, I posted another thread about my car needing a turbo replacement after being tuned for 1,500 miles or so. Well, everything went good and I got the car back with no issues from BMW. I was reading the paperwork and it states, "...waste gate lever broken off on bank 2 turbo." Is this just a freak incident, or has this happened to anyone else?

BTW, I did do a quick search and I didn't really find anything in particular. I'm just curious more than anything.

Broken off? Sounds like it was either an install issue or the result of an impact

I've owned the car since new... Always ran fine and it was never in an accident. I thought it was strange that it said broken off.

The mechanic had to explain to BMW what happened when the turbo plummeted past his hands and onto the floor while removing it from your car.

I have seen them broken off before. Old 6mm style and New 8mm style as well. It happens.
