T-case motor. I'm on my third now with the same symptoms you describe each time.

Doubtful it's the WHOLE tcase going. The actuator motor on mine went at 60k and when I replaced it with a second-hand one it fixed these issues. Order a motor and try it out, they can be had used for $100-150, which is a whole lot less than the ~$600+ price for JUST THE MOTOR at the dealer. I managed to find an entire tcase out of a 328xi with 25k miles for $200 so watch ebay auctions as the pricing for that part is almost tenfold at the dealer.

While its intermittent you could snag an extended warranty for few more bucks to cover yourself and for piece of mind.
