Please help me write a doctors note :-)

ok guys can one of you strapping young lads help me write a doctors note for my dad? he recently got the gastric bypass surgery and is not being admitted back to work. His doctor asked him to put together a letter in which applies to his condition and he would sign it. so i need a letter put together which includes the following:

physiologically and physically stress less environment
quiet, warm, draft free, comfortable permanent workstation. if possible by his choice
no lifting more then 10 lb, no climbing, no bending, no squatting, no running,
no outside works
access of restroom at any time










any help is appreciated guys thanks! my writing skills arent as pristine as a doctors so im hoping someone can help! THANKS!

Let me get this straight. Your doctor wants your dad to write his own note and your doctor will sign it, but your doctor will not write the note himself? What kind of doctor will not write his own patient a note?!

actually i was curious too on why he had to write his own note so i just asked him. he said that he wants to write it himself and have the doc sign off on it lol.

Ok, I guess. I interpreted it as the doc was forcing him to write his own note

If he can have a quiet, warm, draft free, comfortable permanent workstation, if possible by his choice. Due to his current medical condition he can not and shall not lift any object weighing more than 10 lbs. Also no climbing, no bending, no squatting, no running, and no outside work shall be performed. It is also very important that he has access to a restroom at all times.

As the attending physician of Mr. <insert family name here> it is my recommendation that Mr. <insert family name here> return to work, allowing for the limitations outlined below. It is important that Mr. <insert family name here> avoids strenuous physical activity. I therefore recommend the following limitations to his duties; limited physical exertion, including, but not necessarily limited to the avoidance of the following strenuous activities; running, climbing, squatting, bending, or the lifting of objects in excess of ten pounds. Is it that hard to specify what your dad wants?

As a doctor, i do not have the time to list crap out for people. Its either work, no work, or light duty.

If your dad is "after" something then he better write a letter himself.

As another person in the medical field, I'm just saying, he had gastric bypass surgery. Hes still a normal human being. He just eats less.

Hell, isn't the surgery fully laproscopic these days? Damn short recovery time. He is still a normal person and in reality doesn't need any "special treatment". Hes not "physically handicapped".
