I am an MD also. Seems to me, your dad wants EXTRA light duty. The doc did not want to be bothered to get all the *details* of your dads's wishes. So, your dad would rather write the letter himself than try to convince the doc of what to do. So to ensure he gets all the perks he wants in the note, he decided to give it to you.
I would not sign that. He should not have any long-term limitations. The only recovery period would be a matter of days/weeks right after the procedure. After that, I see no reason to have special consideration.
