I treat a lot of OSA patients who are intolerant to a CPAP with mandibular positioning devices. They are custom made oral appliances. I have an ultrasound that I use to diagnose how badly the airway collapses, then I use small bite blocks to reposition the jaw and using the ultrasound I can open the airway and have the appliance made. The improvement I’m able to make is remarkable, and you don’t have to wear a mask. The chin strap won’t work if your nasal airway is compromised. I also evaluate the nasal passage too.

Yes! My DW's dog has given me the 1:00 AM paw tap to the head a few times, so that I would move to the couch to join him far away from the snoring.

Our (read: her) dog will not pay me any attention, unless I'm holding a pork chop or he is scared/sick. It is *extremely* unusual for him to sleep away from her.....

I've tried appliance like that before but sadly they're akin to a plate or braces, and I found over time they moved my teeth to the point where my childhood underbite was threatening to re-occur, that was corrected using a plate years ago.

Were you provided an aligner to wear in the morning after you removed the oral appliance? And was it titrated by having a CT scan or by using the ultrasound like I do.

I can kind of relate. I hate hearing people in the bathroom. I'm planning out floorplans for a house I want to build and one of my must haves is no toilets are along a wall that is shared with a common space or bedroom. I dont want to hear someone dropping nuggets in the damn toilet while I'm eating my cereal or watching TV.

Just put in a door sweep and pay a little extra to have the wall insulated, no issue. My little mini-shitter in my master bedroom (where the toilet is its own room) is setup like that and, despite sharing a wall with the master, you cant hear anything be a faint sound of the flush.

She is mainly worried about:


Despite the fact that I have one of the vent things that is rated for like 4x the space and is only like 1 sone or whatever (aka quiet).

Making progress. Slept till 4:30 this morning, took the device off, and then still managed to sleep some more without it. According to the app, my sleep has gone from 28% and 38% (without the CPAP) to 54%, 64% and then 89% last night. I feel better, although my nose was a little sore at 4:30 which is why I took it off. Hoping that eases, and I can start sleeping through until my dog wakes me for his morning requirements.

So, the Costco girl today. I was driving the Tesla Plaid, which I will post a picture of. I pulled into the parking lot and drove by a cute lady. I said to myself, “she looks yummy”! I got out of the car, she walked by and said, “a Tesla Plaid”? I said, “yeah”. She said, “aren’t those really hard to get right now”? I said, “yeah but I texted Elon and asked him to send one over”. She goes, “oh come on, you did not”. I’m like, “I did”. She goes, “no come on, tell me the truth”. I said, “I’ll bet you lunch I did”. She goes, “only if we take the Plaid”. I’m like, “your on”. So, I showed her our text conversation. She goes, “damn, you weren’t kidding”. She goes, “where we going”? I’m like, In and Out, just down the street”? Now at this point, I didn’t even know her name yet. She got in the car with me and we pulled onto Russell Ranch road. I said, “hang on”, and at the same time I buried the throttle. Her head slammed back against the head rest and she starts yelling, “OMG, OMG, OMG”. I let off the throttle at about 100 and said, “I love it when I make women yell, OMG”! She hit me in the shoulder with her left hand and we both laughed our ass off. I didn’t get her name until after we ordered and sat down. Her name is Shelby. She said her dad raced cars and named her after Carroll. So, we had a nice lunch and nice conversation. I just love it when stuff like that happens!
