My issue is now somewhat reproduce-able. On decelerating the AFR for bank 1 goes to 234.xx and several times Bank 2 AFR stays around 15. After a while and during subsequent decelerations AFR for Bank 2 is stuck at 28. Some research suggest this is a sign of leaky injectors (post #9 http://www.***********.com/showthrea...injector-leaky). However as previously mentioned on Bank 2 I have already replaced spark plugs, injectors, coils, o2 sensor. Any ideas why the AFR on the banks uneven on deceleration?

Sounds like you got at least a marginal injector. Move everything related to fuel from the back 3 to the front 3 and see what happens.

Have you considered swapping the bank 1&2 injectors to see if the problem follows the injectors?

As i previously mentioned, I've replaced Bank 2 injectors with brand new index 12 ones already.

I hear ya. I like to swap when possible, this allows you to deal with a known good/working component. While not likely that all 3 new injectors are bad, it's nice to know for sure that what you are putting in is functional.

I ended up replacing the low pressure fuel sensor even though there were no codes for the sensor. I used the old style sensor (p/n 13537614317) - did not replace with the upgraded line.

As previously mentioned I notice the sensor would read 85-90 PSI upon deceleration just prior the the AFR playing up.
Since replacing the sensor the reading is consistently 72 PSI and around 75 PSI when decelerating. Not going to conclude anything yet but the issue has been gone for 3 weeks now with a combination of highway and stop start driving for about 1500 miles (idle is smooth, no codes, AFR working).
