I would try the worm gear first. Had the drivetrain been making grinding noises prior to that? Mine was getting pretty noisy and still hadn't thrown any codes.
With the worm gear issue due you hear grinding all the time ?

I don't believe so (haven't had that issue myself). Grinding is probably a bearing going out.

if you're referring to the needle thrust bearing going out, it can cause a code if the clutch arm gets spun around too far.

My bearing that went out was the front output bearing. I was getting a pretty good drivetrain vibration and some noise from it and hadn't thrown any codes at that point. Play in the .050" range, so it was significant.

OH, yah that makes sense that that bearing wouldn't throw a code. my misunderstanding

I have the same situation as OP.

I got a '06 330xi 145k miles, got the 3 warning lights on the dash during start up, saw the worm gear on the actuator motor was the main culprit. Removed it and the worm gear looked fine, decided to try a used actuator motor.
Thats when I remembered this thread and to check the positioning of the gear inside the TC, noticed wasn't lined up same as OP. Im going to assume that my needle bearings failed which caused the gear to over rotate as ucdbeindog was so find to explain
