O2 Sensor Faults

I'll just jump right in. I was having bank 2 O2 sensor codes for awhile, "2C32 Lambda control 2" and "2C28 Oxygen sensor before catalytic converter". After replacing the bank 2 O2 sensor and having no affect, I eventually found the issue to be a leaky injector in cylinder 5 that I had replaced at my dealership under warranty.

On my way home from the dealership my SES light popped back on, same two codes as before so I figured I had ruined that O2 sensor as well (the car was running rich and had covered the sensor with what looked like carbon soot).

I replaced the O2 sensor once again hoping for a permanent fix, yet the SES light came back on last night with the exact same lambda control 2 code after about 30 miles of driving after sensor replacement and clearing the codes. I recorded my additive (short term) and multiplicative (long term) fuel trims at idle, additive stayed at 0.0% (which is normal to my knowledge) and my multiplicative in bank 1 was around 6.7-6.8%, and bank 2 was around 10.0-10.1%, which I believe is considered lean. I have catless dp's which I installed myself and have inspected the v-band clamp multiple times and cannot find an exhaust leak yet I always slightly seem to hear one. I'm out of ideas and could use help of any kind, thanks in advance.

Did you replace the turbo seals ?

No I have not, I do have an oil leak from a bad seal though.

No I have not, I do have an oil leak from a bad seal though.
I'd probably fix that given the cost of new turbos.

I assume you replaced the gaskets that connect the downpipes to the exhaust pipes.

Cylinder 5 I think is somewhat common. My injector in cylinder 5 died two months ago after 153,000 miles. I replaced all 6 injectors. All four of my O2 sensors are new with in the last 30,000 miles.

You are running a tune correct ?

Yes I replaced the flanges that connect the downpipes to the mid-pipe of the exhaust. I also am running an MHD Stage 2 tune.

Extremely unlikely that those are the problem...peeps have reused those gaskets over and over again without issue.

How do the rest of the injectors look OP?

Logs?
