'Brake light out' dash warning but lights okay?

Has anyone had this with an e92 335d? 2009 year.

Brake light warning on dash, but the lights are fine?

Yes it's a common problem. The car picks up a change in resistance as bulbs age, even though to the human eye they look OK. Replacement is the only solution I've found...

Is it a bulb replacement? Or a full unit?

Try the bulb first - should cure it.

Good choice, you can always tell if you only replace one.

Brake light warning has now gone, all 4 bulbs were 'burnt' but working, unnoticed the filament connector bit was all smashed, weird!
